Tuition Information for Fall Semest

CONTRACT FOR PRIVATE MUSIC INSTRUCTION

With Louisa LaBerge Creative & Melodies 

I. GENERAL CONDITIONS Lessons will be offered per semester in either group piano lessons format or individual lessons. Students will be scheduled and notified of recurring lesson date and time after registration. 

ii. Students/parents are responsible for providing either themselves or their children an acoustic or digital piano at home to practice on. 

II. ATTENDANCE AND TARDIES  The student and parent understand and agree that lessons must run on schedule in order to not interrupt the flow of other lessons scheduled. If a lesson is missed for a legitimate reason, the student will have one make up lesson per semester. 

III. GENERAL RULES AND PROCEDURES Parents agree not to interfere with lessons by refraining from attending lessons within the same room with the teachers and students unless arranged by the teacher. Parents and students will direct any questions to the teacher. Parents are responsible for reimbursement to teacher for any books or supplies that the teacher buys for the student. 

IV. PRACTICE  Students are expected to practice 30 minutes (more if possible) a day for 5 days a week and complete all written assignments by the next lesson to ensure progress. 

V. PROGRESS REPORTS  You will be mailed a mid and end semester progress report which details your child’s musical progress.  A sample progress report is enclosed.

VI. RECITALS  Students may participate in recitals held throughout the year  Recital Information TDB.

VII. SNOWY WEATHER  If students is unable to attend a lesson due to SNOW or dangerous road conditions, I will reschedule their lesson.

VIII. SUMMER LESSONS   There is currently no enrollment fee to register for Summer lessons. 

You may schedule 4, 6, or 8 summer lessons.  Tuition must be paid for all lessons. 

IX. REGISTRATION AND PAYMENT  A non-refundable payment for the first semester is required at or before the first lesson of the semester to secure a slot. The fee is $25. Registration fee is only collected once per year. Lessons and payment are for a semester commitment.

X. TERMINATION OR RE-REGISTRATION  The student may request to be released from this contract for Private Music Instruction and provide two-weeks notice.  NO REFUNDS WILL BE ALLOWED. 

The teacher reserves the right to terminate a student’s participation for reasons including, but not limited to poor behavior, use of inappropriate language, defiance or disrespect, or not practicing.

Students who do not show progress in playing/singing, in mastering of songs and techniques, and in effort over a three week period will be asked to terminate their lessons immediately. No refunds will be given. This will be explained to students at their first lesson of the semester. 

I have read, received a copy of, and agree to be bound by the terms and conditions of this contract for private music instruction through Louisa LaBerge.
